Weekly Update - Project Completion

Team CACI provided release 1.18 to resolve template selection functionality within the Template Authoring and Template List tabs; all testing defects are closed. We created the Project Summary and Production Integration/Transition Report, finalized the Installation Guide, Technical Manual, and User Manual, and submitted these contract deliverables for project closeout.


Team CACI appreciates the collaboration with the Innovations Coordinator, Business Owner, Stakeholders, and others that fostered project success and innovation graduation of the COMS application in support of VHA Oncology Services.

Weekly Update

Team CACI continued to prepare the final release of Prototype COMS for the VHA Innovations Sandbox and test environments at Durham VAMC and VAPSHCS. Closing the final testing defect, this release will resolve template selection functionality within the Template Authoring and Template List tabs. Our team is finalizing the application for delivery and posting to the Open Source Electronic Health Record Alliance (OSEHRA). We are also preparing submissions for the Installation Guide, Technical Manual, User Manual, and Project Summary and Production Integration/Transition Report required this week.

Weekly Update

Team CACI has isolated the issue with template selection through the Template Authoring and Template List Tabs to resolve for the next COMS release. We are finalizing and testing code to close this final defect reported during User Acceptance Testing. Once released, our team will update the VHA Innovations Sandbox and test environments at Durham VAMC and VAPSHCS. We are providing final updates to the Installation Guide, Technical Manual, and User Manual for end of contract delivery.

Weekly Update

Team CACI finalized and provided software release 1.17 to resolve a type-ahead anomaly within Template Authoring that dropped the first letter of the typed medication and to restore action button operations within the Order Entry Management module that were inoperative only with Internet Explorer. We updated the VHA Innovations Sandbox and Durham VAMC test environments, coordinated with R01 contacts for loading the release for the VAPSHCS test environment, and notified the Innovation Coordinator and Pilot Location Lead Innovators.

Weekly Update

Team CACI received the preliminary results of the VistA Intake Program (VIP) technical review and favorable feedback from VistA Evolution product planning contacts. We are addressing recommendations for additions to the COMS Technical Manual, responding to general inquiries, and attending appropriate meetings to introduce COMS into the live patient environment. Our team is finalizing functionality to resolve user acceptance test defects and provided access to the COMS instance within the VHA Innovations Sandbox for another VHA oncology professional interested in the application.

Weekly Update

Team CACI continued to isolate issues and enhance functionality for the next COMS release. We are finalizing and testing code to resolve a type-ahead anomaly within Template Authoring that drops the first letter of the typed medication and to address inoperative action buttons within the Order Entry Management (OEM) module that only present with Internet Explorer. Our team collaborated with a Durham VAMC test participant/stakeholder to obtain additional insight into the ordering of hydration therapy within chemotherapy regimens. We participated in the first meeting with VistA Intake Program (VIP) contacts as an introduction of COMS for VIP technical review and consideration.

Team CACI finalized and provided software release 1.16 to resolve ‘Remove’ functionality for various Site Configuration panel record entries and address minor display issues. We updated the Durham VAMC test environment, coordinated with R01 contacts for loading the release for the VAPSHCS test environment, and notified the Innovation Coordinator and Pilot Location Lead Innovators. As with previous releases, our team updated the VHA Innovations Sandbox to maintain version control of the prototype in the multiple environments accessed by COMS Stakeholders.

Weekly Update

Team CACI submitted Authority to Operate (ATO) input summarizing efforts since the project inception that contributed to COMS determination as a VistA minor application not requiring a standalone ATO. We were advised by the Innovations Coordinator to not submit input for the Memorandum of Understanding – Interconnection Security Agreement. Our team provided software release 1.15 to resolve the blank fields within the Print Orders display and functionality to restore ‘remove/delete’ capabilities to various panels within Site Configuration. We updated the prototype at Durham VAMC and coordinated with R01 contacts for loading the release for the VAPSHCS test environment.

Weekly Update

Team CACI submitted the two User Acceptance Test (UAT) reports on July 17, 2015 to document pilot location test activities, conclusions, and recommendations. We are continuing the preparation of input for the Memorandum of Understanding – Interconnection Security Agreement and documentation of contributions to various precursor products leading to COMS inclusion as a minor VistA application subject to the existing Authority to Operate. Our team is finalizing the next release to resolve a defect observed during UAT activities; we will update the prototype within the Innovations Sandbox and at both pilot locations.

Weekly Update

Team CACI conducted the User Acceptance Test (UAT) debrief session with test participants on July 10, 2015. We presented all feedback – including user tips, defects and resolution, and recommendations for future enhancement – and next steps for the innovation. We will include all feedback in the UAT reports and highlight those items identified for Stakeholder consideration of further enhancement. Our team assembled and forwarded all requested information to the Innovations Coordinator for the Product Planning Document (PPD) and VistA Intake Program (VIP) technical review to facilitate use in the live patient production environment for pilot locations and future deployment sites.

Team CACI concluded user acceptance testing support at the two pilot locations and responded to test participant final feedback. We initiated drafts of the User Acceptance Test reports and updates to the Technical Manual and Installation Guide. Our team continued refinement of the COMS Prototype to resolve reported or discovered bugs and further enhance user experience. We participated in the Innovations Coordinator meeting for the Product Planning Document (PPD) and began assembling information requested for the PPD and VistA Intake Program (VIP) technical review to facilitate use in the live patient production environment for pilot locations and beyond.

Team CACI continued to support user acceptance testing at Durham VAMC and VAPSHCS. We provided software release 1.14 to resolve the Remove Reference & Remove Reminder bugs and to provide the ability to retrieve medication names by either generic or trade name. We updated the prototype at Durham VAMC and provided information to R01 contacts for updating the VAPSHCS test environment. Our team conducted a trouble shooting session with a VAPSHCS test participant to observe a reported issue with selective retrieval of patients then coordinated with R01 contacts to obtain database files from the test environment. We reviewed the files, detected anomalies, provided remedy, confirmed resolution with R01 contacts, and notified the VAPSHCS lead innovator.

Team CACI provided software release 1.13 to resolve reported defects with medication edits, erroneous substitution of IENs for medication names, display of relevant IV fluid information, and availability of documentation lists and contents. We updated the prototype at Durham VAMC and provided information to R01 contacts for updating the VAPSHCS test environment. Our team informed the Innovations Coordinator of pilot location requests to extend the testing period and accommodated the approved extension through July 3, 2015. We updated the User Acceptance Test Plan and debrief session to reflect the extended testing period.

Weekly Update

Team CACI continued to support user acceptance testing at pilot locations and conducted two trouble shooting sessions with test participants. We met with VAPSHCS stakeholders to confirm pharmacy synchronization functionality resolved initial medication selection concerns. Our team met with a Durham VAMC test participant and discussed issues regarding selection of multiple patients with the same patient ID (this population does not exist within the VHA Innovations Sandbox), presentation of medications by generic vice trade name, and medication name/route of administration display following the edit process. We also discussed new enhancement requests for consideration and preferences for scrubbing test data as part of the pre-production environment setup.

Team CACI finalized COMS Release 1.10 for user acceptance testing at pilot locations June 8 – 19, 2015. We updated the prototype at Durham VAMC and provided information to R01 contacts for updating the VAPSHCS test environment. Our team provided an updated User Manual and all six Use Case & Procedures documents for test participant use. As requested by the Innovations Coordinator, we met with R01 and Durham VAMC OI&T contacts to prepare for introduction of the COMS Prototype into the live patient clinical environment at both pilot locations following conclusion of testing activities.

Team CACI provided a testing round debrief with test participants and continued functionality refinements to enhance the user experience within tailored, local environments. We are updating Use Case and Procedures documents and the User Manual to reflect these refined capabilities, including synchronizing medication lists. Our team resolved a missing file issue that impacted the May 28, 2015 debriefing session within the Durham VAMC test environment and pushed the patch to pilot locations. We are finalizing miscellaneous refinements for release to pilot locations for User Acceptance Testing (UAT) activities June 8 – 19, 2015.

Weekly Update

Team CACI finalized functionality and coordinated the release to synchronize medication lists between the COMS application and associated VistA instance. We conducted a trouble shooting session with the primary pharmacy test participant at Durham VAMC and determined the retrieval/display of the results was not correct. Our team reviewed error logs, identified the root cause, resolved the defect, and pushed the software patch to pilot locations. We have confirmed functionality in the Durham test environment and are coordinating with VAPSHCS for a pharmacy test participant to process the medication synchronization for that environment. We have extended Testing Round II into the first week of June to permit further assessment of the prototype at pilot locations.

Team CACI conducted two troubleshooting sessions with VAPSHCS test participants and resolved a connectivity issue between COMS and the associated VistA Test instance introduced during loading of the last release. We have isolated an issue regarding medication selection and initiated coordination action for appropriate staff supporting the test locations to execute the remedy script to enable access to the appropriate drug files. We met with the Innovations Coordinator to provide status update on pilot location testing to include these issues, requests for access to test environments to enhance trouble shooting, overall user participation, and feedback received to date.

Team CACI finalized, tested, and released software refinements for Government Testing Round II. New functionality includes capabilities to specify the need/support of co-signatures for preceptee users and to indicate concurrent radiation therapy for patients when applying a template. We coordinated loading of COMS Prototype Release 1.4 with OI&T staff supporting pilot location testing at Durham VAMC and VAPSHCS. We finalized and provided updates to the User Manual and six Use Case & Procedures documents for clinical/miscellaneous functionality.

Weekly Update

Team CACI integrated the patient retrieval solution into the user interface of the COMS application to enable all appropriate users – regardless of role-based access – to select and retrieve patients from associated VistA instances. We have enhanced functionality to permit “in-training” users, such as oncology fellows, to initiate actions that require preceptor co-signature. Our team has also finalized functionality to designate concurrent radiation therapy for applicable patients when applying templates for chemotherapy regimens. We are performing end-to-end, pre-release testing prior to the next software release for Government Testing Round II.
